• Completed RinkGuard Application signed by the insured including complete information specific to rink revenue. 
• Certificates of insurance with additional insured status for vendors, concessionaires, special events when available.  
• Include verification of USA Hockey membership for youth hockey activities.
• Copies of releases used if any.
  

Please complete the appropriate supplemental applications if the following activities are conducted:

• Fireworks
• Day Care Center/Nursery
• Concerts
• Climbing Walls

• Completed Accord applications, signed by the insured, including detailed property information to include construction, occupancy, square footage, protection and age of respective buildings to be insured.
• 5 years of recently valued insurance company loss runs
• Details on any losses over $25,000
• Expiring premiums and target premiums for all lines of coverage being submitted
• Web site address, brochures and photos if available    

If not yet appointed with Willis of New Hampshire, Inc., please include the following: 

• Completed and signed BROKERAGE AGREEMENT
• Proof of agency E&O showing a minimum of $1,000,000 limit per occurrence.
• Copy of licenses in all States you plan to broker the RinkGuard Program
